Registering with the Practice

We welcome new patients, and would like the opportunity to provide for your health care needs. If you are moving into our area and wish to register with our Practice, please come into Reception at any one of our sites where we will ask you to complete our registration form and new patient questionnaire.

You will be asked to provide ID in the form of a passport or driving licence and utility bill to verify your current address. We need to register you before you can see a clinician or ask for a prescription.

You will be given a Registration form and a new Patient Questionnaire to complete and hand back to a member of staff. Alternatively you can print off the below forms and bring them into the practice for a faster registration. Our staff will inform you who your “named GP” is. It is important to note that you can be seen by any GP/clinician working at the practice, and can ask to change your “named GP” at any time.

It is hoped that you will let us have your email address, if you have one, and consent to our using it for non-clinical purposes such as newsletters about the Practice. You may then be offered an appointment with a nurse if you wish to have a new patient check and you will be required to see a doctor if you are on regular medication.

COVID-19 – Please note that we are trying to minimise the footfall into the surgery wherever possible at this time. Please download a copy of our registration form and new patient questionnaire and complete in full before sending to the surgery. Please ensure you have included accurate contact details. We will call you if we require any additional information to complete your registration.
