Mendip Vale Medical Group

Mendip Vale Medical Group work with Concord Medical Centre as a PCN (Primary Care Network). This an NHS collaboration between the surgeries, with the aim to support and connect with our local community. We provide quality NHS healthcare services to over 100,000 patients in our two PCNs.

Our surgeries are located in Bristol (Horfield, Sea Mills, Stoke Bishop and Southmead), North Somerset (Langford, Yatton, Congresbury, Clevedon, and Worle), and South Gloucestershire (Patchway). We all work together to give our patients a choice over where they visit, who they see and the services available. See below the surgeries that are part of our network:

What is a Primary Care Network?

A Primary Care Network (PCN) brings surgeries together with local services to look after patients across an allocated area. PCNs are able to provide wider care for the people and community. Our North Somerset surgeries form a PCN. Our Bristol and South Gloucestershire surgeries along with Concord Medical Practice are a PCN for our Bristol and South Gloucestershire patients. Working together this enables us to improve standards across our locations, services, teams and pharmacies.

As a PCN, we make sure patients have access to quality NHS services across our surgeries. We bring NHS services and local services together, such as community, mental health, social care and volunteering, to look after local patients and provide more integrated health and social care.
