We welcome new patients, and would welcome the opportunity to provide for your health care needs.
If you are moving into our area and wish to register with our Practice, please come in to Reception at any one of our surgeries where we will ask you to complete a form. You will be asked to provide ID in the form of a passport or driving licence and utility bill to verify your current address please. We need to register you before you can see a doctor or ask for a prescription.
You will be given a New Patient Questionnaire to complete and hand back to the receptionist. The receptionist will inform you who your "named GP" is. It is important to note that you can be seen by any GP working at the practice, and can indeed ask to change your named GP at any time. It is hoped that you will let us have your email address, if you have one, and consent to our using it for non-clinical purposes such as newsletters about the Practice.
You may then be offered an appointment with a nurse if you wish to have a new patient check and you will be required to see a doctor if you are on regular medication.
Alternatively you may print off a registration form , by clicking the link below. Please complete and bring with you on your first visit to the practice, along with the ID as outlined above.

A Patient Guide to GP Services
Mendip Vale Medical Practice is committed to providing safe and effective care for all of our patients. If you have any specific preferences that would assist in the way the practice and staff communicate with you please let us know.
We can take steps to ensure we are communicating effectively with you - your local Surgery Manager will be delighted to assist.
There are a number of ways we can provide our services to best meet your needs.
At each of our sites we have a hearing loop available, this is situated at the reception desk and as it is portable can be moved to consultation rooms.
If sign language would be helpful we can arrange with Action on Hearing Loss to provide British Sign Language (BSL) during your consultation.
We can provide information in large print and our website has a tab at the bottom to change the view, adaptable to many different web browsers.

These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.
Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups. Open the leaflets in one of the following languages
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
